• Completed V55/5 to me with proof of ID (as above)
• Cheque for £55 payable to the DVLA
• Large stamped envelope addressed to the DVLA. Large letter stamp please. (Ensure photos have been enclosed).

After confirming the information provided is correct (this may entail a visit from myself) .

I will:

• Send a Letter of Support/Dating Certificate on International Harvester Heritage Association headed paper and will forward the information to the DVLA in the SAE envelope provided by you. You will also receive a copy of the Dating Certificate for your file (if you request it).
